Your data is intended exclusively for Dr Serdal Sanak. It is in no way sold, rented or transmitted to third parties for commercial purposes.
The form is technically managed by our host Netlify, Inc. (United States), acting as a processor. Data transfers to the United States are framed by the EU-US Data Privacy Framework, to which Netlify has adhered, guaranteeing a level of protection in line with European requirements.
Appropriate technical and organisational measures are in place to protect your data against loss, unauthorised access, disclosure or alteration: secure connection (HTTPS), restricted access to data, hosting with a GDPR-compliant provider.
You have a right of access, rectification, erasure, objection, restriction and portability concerning your data. You can also withdraw your consent at any time, without affecting the lawfulness of processing carried out beforehand.

The preoperative questionnaire, offered to patients with an appointment for an exam, collects information about your health: eye symptoms, contact lens wear, treatments, medical history, lifestyle and expectations. It asks for no surname, date of birth, email address or phone number. Only the first name, age, date and site of the appointment are used by Dr Sanak to match the answers with his diary.
Purpose: to prepare the preoperative exam consultation. A dry eye score and points of attention are calculated to guide the discussion. No decision is taken automatically; everything is discussed at the consultation after the examination.
Legal basis: your explicit consent (articles 6.1.a and 9.2.a of the GDPR), given by ticking the box provided before sending.
No storage: the answers are stored neither on this site nor with the hosting provider. They are turned into a PDF document emailed to Dr Sanak, then cleared from the server's memory. The email is sent through Brevo (Sendinblue SAS, Paris), whose servers are located in the European Union. To limit abuse, an encrypted fingerprint of the IP address is kept for at most two hours, with no way to recover the address.

Retention at the practice: the document is added to your medical file if a consultation takes place, and kept under the rules applicable to patient records (Belgian law of 22 August 2002 on patients' rights). If no consultation takes place, it is deleted within 12 months.
